I had disputed an account and got it removed from my report with Experian. However, when I checked recently, I saw that it has been re-inserted, and I didn’t even know anything about it. Isn’t the CA required to notify about this to me? I checked other reports too, and they’re all clear. Will I need to dispute the same account again and again?

Yes, you’ll have to dispute it again. Call them up or send them a letter stating the issue and requesting immediate removal of the account. And you’re right, the agency was supposed to inform you about re-insertion of the account, which had been previously disputed and deleted. Since they didn’t, it’s violation of the FCRA.
